The form below should be used to submit only students who were missed entirely or need additional days funded for the 2020-21 school year. If submitting additional days submit the TOTAL remote learning days for that month and not just the additional ones. Do NOT submit students who were missed for SUMMER benefits as that collection is closed. If you have more than 10 students to submit, please contact support at pebt-support@ohio-k12.help.
