Blended and Remote Learning

Senate Bill 229 requires city, exempted village, and local school districts, joint vocational school districts, community schools, STEM schools and chartered nonpublic schools to report the total number of students engaged in blended or remote learning by grade level and the total number of students with disabilities engaged in blended or remote learning during the 2021-2022 school year.  Districts and schools also are required to report the number of students participating in blended learning for each quarter of the 2021-2022 school year and the duration of such participation. These data collected in the form below will be used in a legislative report sent to members of the Ohio General Assembly in mid-May.  

This data collection should only be completed by a district or school that:

  • Submitted a blended learning declaration to the Department of Education for the 2021-2022 school year; or
  • Adopted a resolution for the 2021-2022 school year to continue using the remote learning plan submitted to the Department for the 2020-2021 school year.

This data collection must be completed in a single session. There is no option to save data and return to complete at a later time. In preparation for this survey, please collect all requested data.
